Aveo invites a Care Partner (Clinical) to join our NSW home care teams, supporting residents in their own homes within Aveo communities. You will assess needs, craft personalised care plans, and coordinate services to help residents live independently for longer.
As an autonomous, community-based RN, you’ll rely on your clinical judgement, communication skills, and collaboration with GPs and allied health to deliver high-quality care, while using Alayacare and MS Office on mobile devices.
