Amplar Health is seeking a Clinical Nurse to join our sub-acute Transition Care Service in Walkerville on a full time basis. You will provide clinical leadership and expert nursing care to patients with complex needs within a multidisciplinary team.
You will coordinate patient care across settings, support governance and service coordination, and contribute to quality improvement and system improvement. The role requires 3+ years post-registration, unrestricted AHPRA, rostering & budgeting
