Helping Hand Aged Care is seeking a Care Partner to support older people to live safely and independently at home. You will coordinate services, co-design tailored care plans and advocate for customers’ rights in partnership with families and internal teams.
You’ll work in Salisbury South in a permanent, full time role, contributing to wellbeing and seamless transitions between services while respecting customers’ preferences and choices.
