MedicalJobsAustralia is seeking a Registered Nurse to join the Transitional Aged Care Program (TACP) within Western NSW Local Health District. The role supports older people to regain independence after a hospital stay through multidisciplinary teamwork in a community setting.

You will deliver tailored clinical and non-clinical services to boost physical and cognitive functioning, promote reablement, and reduce readmissions, with a 38-hour week and a focus on protected, home-based care in Dubbo, NSW.
