Life Stream Australia has an chance for a supportive, values driven and engaging Coordinator to coordinate a team in Brisbane and Canberra to support Participants with an intellectual disability to develop friendships, be active, involved in community and centre based activities and work towards NDIS goals. As part of Team Life Stream, we ask staff to demonstrate the values of LIVES WITH CHOICE.

This is a part time position, based at Mt Gravatt East starting with 30 hours per week. As you grow the service, your hours or support team can increase. You will be coordinating approximately 12 Support Workers, managing the roster and liaising with Participants and Carers across individualised support (community access and in-home support), weekend group leisure programs, holidays and STA. You are required to plan a quarterly leisure and holiday program and manage the bookings and program logistics.



You will also be required to provide On Call support via phone for programs outside of office hours.

Life Stream Australia is a NDIS registered service provider and as such, requires employees to have a NDIS Worker Screening Card (or capacity to get this). Employees are required to have (or have the capacity to get), their first aid certificate (including CPR), a smart phone and a reliable vehicle.

Training modules will be shared at the commencement of employment and there will be an prospect to be involved in a state wide support network for career development.
