Coast & Country Primary Care (CCPC) is seeking a Coordinator of Supports/Recovery Coach in Tuggerah, NSW. This hybrid role centers on coordinating supports for NDIS participants and building robust relationships with local providers to help participants achieve goals.

You will monitor progress, support recovery planning, and ensure services align with the NDIS Code of Conduct, safeguarding, and participant choice. Some travel and a adaptable, community-focused approach are required.
