American Heart Association BLS certification required for all direct patient care personnel ACLS certification required for RNs in CCU, ED, PACU, MICU, SICU (included here as PCU is a progressive care unit) State nursing license verification must be dated within 30 days of start Trinity Specialty Specific Competency Testing must be completed within a year Trinity Code of Conduct Form must be signed Trinity I9 form completed by agency Confidentiality and Security Agreement signed Integrity and Audit Services Form signed prior to start Trinity Criminal Background Check within last 7 years, dated within 30 days of start Education verification of highest academic degree required EPLS and GSA/OIG exclusion reports dated within past 30 days Joint Commission CORE Exam within a year NSO report dated within 30 days of start,
