We are seeking a permanent Care Partner - Enrolled Nurse or Care Partner – Clinically Qualified Registered Nurse to join our Aged Care team in Launceston.
The position is permanent, full-time (76 hours fortnightly) or part-time (60.8 hours fortnightly) arrangements can be made for the right candidate.

About the role:
The Care Partner Enrolled Nurse/Registered Nurse provides clinical care, care planning and service coordination to help older adults remain safe, independent and well supported at home. Using a rights-based, person-centred approach, the role works with participants, families, support networks and interdisciplinary teams to deliver inclusive care aligned with each participant's goals, preferences and needs. The role coordinates and oversees aged care supports, working with clinically trained Care Partners when specialised clinical care or complex planning is required.
What you will be doing in this role:
Plan, review and coordinate personalised care, services and budgets.
Deliver clinical assessments, nursing care and case management under supervision.
Monitor wellbeing,



changing needs, risks and care outcomes.
Work with clinical, allied health and service partners to support complex care.
Coordinate reviews, transitions, case conferences and access to appropriate services.
Support participant choice, self-management, health literacy and carer engagement.
Maintain accurate records and meet quality, governance and safety requirements.
Contribute to quality improvement, incident management, supervision and team development.
What we can offer you:
$95,934 to $111,782 plus super + salary sacrifice (full-time equivalent) depending on qualifications and experience;

The successful applicant will be required to:
provide a current Tasmanian Working with Vulnerable People Registration (employment/volunteer category);
present a unsupervised contact with children or vulnerable people type National Police Check (no older than 12 months;




provide an International Police Check if you have resided for 12 months or more outside of Australia after the age of 16;
provide a satisfactory Anglicare assessed medical;
provide a current provisional or full Australian Driver Licence.
Diploma of Nursing and current APHRA registration.
Demonstrated experience in care coordination, case management or similar roles in aged care, health, or disability services.
On commencement the successful applicant will be required to undertake our structured Safeguarding Children Training program.
