As a full time Case Manager based in Baulkham Hills with Lifestyle Solutions, you'll play a vital role in supporting children, young people, families, and carers through strength-based, person-centred case management.
You'll work collaboratively to deliver meaningful outcomes by developing tailored support plans, coordinating services, and advocating for the young people in your care.If you're passionate about supporting children and young people to reach their full potential, we'd love to hear from you!
