About the role

Saltire Care Pty Ltd is seeking a dedicated and compassionate Support Coordinator to join our team in Rockhampton, QLD. This is a full-time position that plays a vital role in coordinating and delivering support services to individuals with disabilities and aged care needs. As a Support Coordinator, you will be at the heart of our mission to enable people to live independently and participate fully in their communities. This role is essential to ensuring our clients receive personalised, high-quality support that enhances their quality of life and promotes their independence.

What you'll be doing

- Assessing clients' support needs and developing individualised care plans that reflect their goals, preferences and aspirations

- Coordinating a range of support services to meet clients' disability and aged care requirements, working collaboratively with service providers and stakeholders

- Providing direct support and assistance to clients in daily living activities, personal care, mobility and community participation

- Building and maintaining strong relationships with clients and their families, ensuring open communication and regular feedback

- Monitoring client progress and wellbeing, adjusting support arrangements as needed to ensure optimal outcomes

- Maintaining accurate and confidential client records and documentation in compliance with regulatory requirements

- Liaising with medical professionals, allied health providers and other community services to ensure coordinated care

- Advocating for clients' rights and ensuring they have access to appropriate support and resources





- Contributing to team meetings and continuous improvement initiatives to enhance service quality

What we're looking for

- Relevant qualifications in disability support, aged care, social work or a related field, or substantial equivalent experience in community services

- Knowledge of disability support frameworks, aged care standards and relevant legislation including the National Disability Insurance Scheme (NDIS)

- Demonstrated experience working with people with disabilities and/or older adults in a support or care coordination role

- Strong assessment and care planning skills with the ability to develop person-centred support strategies

- Excellent communication and interpersonal skills with the ability to build rapport and trust with clients, families and service partners

- Ability to work independently and as part of a team, showing initiative and problem-solving capabilities

- Current First Aid and CPR certification (or willingness to obtain)

- A valid Australian driver's licence and access to reliable transport for community visits

- A commitment to safeguarding principles, duty of care and maintaining client confidentiality

- Resilience, empathy and a genuine passion for supporting vulnerable people to achieve their goals
