The Prospect

As a Care Coordinator, you’ll be the engine behind outstanding care delivery. You’ll take ownership of client services, support-worker coordination and day-to-day service outcomes. We’re looking for someone with experience in aged care or the Support at Home (SAH) program who understands the importance of delivering services that align with client needs, budgets and care plans.

What You’ll Do

- Coordinate client bookings and service requests accurately and efficiently.
- Build rosters that meet client needs, budgets and SCHADS requirements.
- Match clients with suitable support workers based on skills, preferences and availability.
- Keep clients, families,



support workers and internal teams informed.
- Respond to roster changes, incidents and complaints with calm confidence.
- Maintain accurate records and follow through until issues are resolved.
- Build trusted relationships that support positive client outcomes.

About You

You’ll bring

- Experience in aged care, including exposure to the Support at Home program or home-care service delivery.
- A relevant qualification in Health, Aged Care, Disability or Community Services.
- Strong communication and relationship-building skills.
- The ability to solve problems and make sound decisions under pressure.
- Excellent organisation and attention to detail.
- Confidence managing competing priorities in a fast-moving environment.
- Knowledge of care plans, client budgets and the SCHADS Award will be highly regarded.
