BDAC was founded to represent and provide services to Aboriginal and Torres Strait Islander people living on Djaara Country.

About the Role The Chronic Care Coordinator is responsible for delivering a culturally safe, intensive short-term care coordination and navigation program for Aboriginal and Torres Strait Islander Community Members living with cardiovascular disease, respiratory disease and/or diabetes who are at increased risk of hospitalisation, disease progression, fragmented care or disengagement from health services. Working within BDAC's multidisciplinary Chronic Disease Management Team, the position delivers focused, short-term interventions aimed at stabilising health needs, enhancing self-management, and supporting clients to achieve their identified health goals. The role focuses on identifying Community Members who are experiencing, or are at risk of experiencing, deterioration in their health, responding to care gaps, supporting transitions of care, stabilising health needs, strengthening self-management capacity, and connecting clients with ongoing supports.

Unlike traditional long-term case management roles, the Chronic Care Coordinator delivers intensive interventions designed to achieve specific health goals, reduce barriers to care,



improve coordination between health providers and reduce potentially preventable hospital admissions and emergency department presentations.

The position works proactively across primary care, acute care and community services to ensure Community Members receive timely, coordinated and culturally safe healthcare. This includes hospital discharge follow-up, outreach support, multidisciplinary care planning, appointment coordination, referral management, chronic disease education and transition planning to discharge or longer-term programs.

The role operates within BDAC's Aboriginal Community Controlled Health model and applies relationship-based, strengths-focused and culturally responsive approaches that promote self-determination, holistic wellbeing and improved health outcomes for Community.

What you will bring

- Demonstrated knowledge, skills, and experience in chronic disease management.
- Demonstrated experience in case management and care coordination.
- Demonstrated knowledge and understanding of Aboriginal culture, community priorities, and the Aboriginal Community Controlled Health Organisation (ACCHO) model of care, with a commitment to providing culturally secure and responsive services.
- Demonstrated experience working within community health,



primary health care, general practice/medical clinic, and/or acute healthcare settings.
- Proven ability to work collaboratively with multidisciplinary teams and engage effectively with a broad range of stakeholders.
- Demonstrated experience supporting patients with chronic disease, complex health needs, and co-morbidities, with a focus on preventative health, self-management, and reducing avoidable hospital admissions.
- Highly developed organisational, communication, and interpersonal skills, with the ability to build strong relationships with clients, families, community members, and service providers.
- Proven adaptability, flexibility, and problem-solving skills, with the ability to respond effectively to changing priorities, service delivery requirements, and community needs while maintaining high-quality care.
- Demonstrated commitment to continuous quality improvement, professional development, and evidence-based practice.

Education, Training and/or Competencies

Mandatory

- Bachelor of Nursing – Registered Nurse with the Australian Health Practitioner Regulation Agency
- Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care -Registered with the Australian Health Practitioner Regulation Agency

Preferred / Desired

- Previous experience working with Aboriginal and/or Torres Strait Islander community members or in an Aboriginal organisation.
