About the role

Better n Beyond is seeking an experienced and compassionate Registered Nurse to join our team as a Clinical Care Coordinator. This is a senior clinical and operational role supporting participants across Supported Independent Living, community access, in-home and complex-care services. You will provide clinical leadership, oversee secure support practices and help Care Coordinators and frontline workers deliver consistent, person-centred care.

The role combines clinical coordination, staff development, quality assurance and practical service oversight. It suits an RN who enjoys working closely with participants, workers, families, health professionals and other stakeholders.

Key responsibilities

- Complete RN assessments and respond appropriately to changes or deterioration in participants' health.
- Develop, review and maintain participant health plans, clinical instructions and escalation pathways.
- Oversee medication management, PRN protocols, infection control and high-intensity supports.
- Coordinate complex participant onboarding, hospital discharge and service transitions.
- Conduct regular visits to SIL houses and other service locations.
- Review medication records, health data, incident reports, clinical documentation and identified discrepancies.
- Monitor participant risks, safeguarding concerns and required corrective actions.
- Guide and supervise Care Coordinators, House Coordinators, Team Leaders and support workers.
- Deliver staff education, training and assess practical competency within professional scope.
- Monitor incident reports and medical alerts, escalate concerns where appropriate,



and ensure clear documentation in line with NDIS and internal standards.
- Maintain organisational compliance and contribute to internal audits, quality reviews, and continuous improvement initiatives.

About you

- Current unrestricted registration as a Registered Nurse with AHPRA.
- A recognised nursing qualification.
- Minimum 2 years of nursing and coordination post-registration experience, preferably in disability, aged care, community health, complex care or clinical coordination.
- Strong knowledge of medication safety, clinical deterioration, infection control and high-intensity support.
- Experience providing clinical leadership, staff training and practical competency assessment.
- Strong documentation, communication and stakeholder-management skills.
- The ability to balance participant choice, dignity, clinical risk and duty of care.
- Current First Aid and CPR certification.
- Current NDIS Worker Screening Clearance or willingness to learn.
- A current unrestricted driver licence.
