Client Centred Home Care (CCHC) provides community nursing and support services across South-East Queensland & Northern NSW. We take a continuity-of-care approach - clients are supported by consistent, familiar teams rather than rotating agency staff.
We're looking for a Registered Nurse Case Manager to join our Head Office team in Southport, Monday–Friday, 8am–4pm, reporting to the Clinical Care Manager.

About the Role
You'll provide clinical oversight and case management for a diverse client group, including many people living with disability or complex neurological conditions such as spinal cord injury, MND, MS, Huntington's, cerebral palsy, stroke and dementia. You'll also mentor the in-home nursing and support teams working with those clients.
The role combines office-based case management with community work across the Gold Coast and Logan, with occasional travel across South-East Queensland, including Brisbane and Ipswich. Responsibilities include:
- Client onboarding, assessments and care planning
- Direct nursing care — wound management, catheter care, insulin and medication administration, dysphagia support
- Comprehensive health, wound, diabetes and continence assessments and reporting
- Training and mentoring support workers and community nurses

What You'll Bring
- Diploma or Degree in Nursing with full, unrestricted AHPRA registration
- Experience as a Case Manager managing a caseload of clients
- Current CPR and First Aid certificate
- Ability to assess, plan and deliver client-centred care within your scope of practice
- Solid communication skills and the ability to work both autonomously and within a team
- Sound time management, organisational and problem-solving skills
- Confidence with IT systems and attention to detail
- Blue Card and NDIS Worker Screening Clearance (or willingness to obtain), Australian National Police Check, driver's licence, and legal working rights in Australia
