About the Role

We are seeking a motivated and compassionate Community-Based Physiotherapist to join our multidisciplinary team on a part-time basis, servicing Dandenong and surrounding suburbs.

This is predominantly a mobile/community role involving home visits and community-based physiotherapy services, including Residential Aged Care facilities.

The position offers flexibility and autonomy, supported by an established multidisciplinary allied health team.

Key Responsibilities The successful Physiotherapist will:

- Conduct comprehensive physiotherapy assessments in clients' homes, community settings and

Residential Aged Care facilities.
- Develop individualised, evidence-based treatment and rehabilitation programs based on functional needs and client goals.
- Provide physiotherapy intervention for clients with musculoskeletal, neurological, age-related and chronic health conditions.
- Develop and prescribe individualised home exercise programs.
- Assess mobility, balance, strength,



transfers, gait and functional capacity.
- Conduct falls-risk assessments and develop practical falls prevention strategies.
- Provide gait retraining, balance rehabilitation, strengthening and functional reconditioning.
- Assess and provide recommendations for mobility aids, walking equipment and other relevant assistive technology.
- Provide pain-management strategies and education regarding safe movement, injury prevention and self-management.
- Educate clients, family members and carers regarding exercise, mobility, transfers and ongoing management.
- Monitor clinical progress and modify treatment plans according to outcomes.
- Liaise with SAH care partners, NDIS support coordinators, GPs, nursing staff, residential aged care teams and other allied health professionals as required.
- Prepare high-quality clinical documentation, progress reports, equipment recommendations and other relevant reports.
- Maintain documentation consistent with professional, organisational, NDIS, aged care and applicable funding requirements.
- Participate in multidisciplinary communication and contribute to coordinated client care.

Qualifications & Requirements

Essential:

- Bachelor of Physiotherapy or equivalent recognised physiotherapy qualification.
- Current registration as a Physiotherapist with AHPRA.
- Robust clinical assessment, treatment planning and clinical reasoning skills.
- Ability to manage clients presenting with musculoskeletal, neurological, mobility and chronic health conditions.
- Strong understanding of person-centred and goal-directed care.
- Excellent written and verbal communication skills.




- Ability to work autonomously in community environments while remaining an active member of a multidisciplinary team.
- Strong organisational, caseload and time-management skills.
- Ability to maintain accurate electronic clinical records and professional documentation.
- Current Australian driver's licence.
- Reliable vehicle and willingness to travel between clients within the designated service area.
- Commitment to professional development, ethical practice, confidentiality and evidence-based physiotherapy.
