About The Client Our client is a lovely lady in her 50s living with Progressive Multiple Sclerosis alongside Type 2 Diabetes. She is warm and easy to get along with when supported by calm, familiar staff who take the time to understand her routine. She is physically mobile with a 4-wheeled frame and independent with most transfers, but she needs cognitive support, prompting and reorientation across her day, along with hands-on assistance with medication, insulin and continence care.

She does best with consistent, patient support workers who communicate calmly, redirect gently, and understand that her behaviours are a direct expression of her condition rather than a reflection of who she is. When approached the right way, she is easily redirected and settles quickly.

About The Role

- Personal care including showering, dressing, grooming and continence support, delivered with dignity and warmth.
- Prompting and cognitive support across daily routines to help her stay engaged and oriented.
- Medication administration including insulin and blood glucose level monitoring.
- Meal preparation and supervision, supporting her to follow her diabetic diet.
- 1:1 community access, transport and social participation.
- Calm,



respectful de-escalation using verbal redirection and reorientation.
- Accurate documentation of shift notes, medication records and any incidents in line with NDIS Practice Standards.

About You

- Minimum 2 plus years of experience in the disability sector.
- Experience working in behaviour support environments, with exposure to restrictive practices highly regarded.
- Experience supporting clients with progressive neurological conditions (MS, Parkinson's, dementia) highly desirable.
- Confidence with medication administration, ideally including insulin and BGL monitoring.
- Robust de-escalation skills, with the ability to respond calmly and safely to behaviors of concern in line with Behavior Support Plans.
- Able to follow structured routines and deliver consistent, person-centered support that promotes participant safety and independence
- Competent in documentation and data collection, including incident reporting, progress notes, and ABC charting
- Understanding of regulatory and safeguarding requirements within the NDIS framework (desirable)
- Minimum Certificate III Disability or similar/higher.
- NSW Driver’s License (or similar) plus own car is desirable.
