We are seeking a Registered Community Nurse who shares our values and is passionate about making a meaningful difference in the lives of older adults.

Position Overview The Registered Community Nurse provides clinical care, assessments, and ongoing monitoring for clients living at home. This role requires strong clinical judgement, excellent communication skills, and a commitment to delivering care that honours each client’s dignity, safety, and wellbeing.

You will work collaboratively with clients, families, support workers, GPs, allied health professionals, and hospitals to ensure seamless, coordinated care.

Key Responsibilities

- Conduct comprehensive nursing assessments and develop individualised care plans
- Provide wound care, medication management, catheter care (IDC/SPC), continence support, and chronic disease monitoring
- Complete risk assessments, health monitoring, and wellbeing checks
- Support hospital‑to‑home transitions and short‑term restorative care
- Identify changes in client condition and escalate appropriately




- Provide education and guidance to clients, families, and care staff
- Collaborate with multidisciplinary teams to ensure coordinated care
- Maintain accurate, timely, and compliant clinical documentation
- Uphold the Aged Care Quality Standards, clinical governance, and organisational policies
- Contribute to quality improvement activities and safe care practices

Prerequisites & Essential Requirements

- Current registration with AHPRA as a Registered Nurse (RN)
- Experience in community nursing, aged care, chronic disease management, or similar clinical settings
- Current First Aid & CPR
- National Police Check (or willingness to obtain)
- Valid driver’s licence and reliable, insured vehicle
- Strong clinical judgement and ability to work autonomously
- Excellent communication, documentation, and interpersonal skills
- Commitment to person‑centred care, dignity, respect, and safety
- Ability to maintain professional boundaries and confidentiality

Desirable Criteria

- Experience in wound management, dementia care, palliative care, or transitional care
- Knowledge of Aged Care Quality Standards and community‑based care frameworks
