We're recruiting on behalf of our client, a charitable organisation on a mission to inspire healthy and happy living across generations and communities. Their Support at Home Team helps clients stay independent, protected and connected in their own homes — and they're looking for a Senior Clinical Care Partner to lead the clinical direction of that team.
This is a hands-on leadership role for a Registered Nurse who wants genuine autonomy, a team to develop, and the chance to shape how care is delivered — not just deliver it.
About the role
As Senior Clinical Care Partner, you'll provide clinical leadership and oversight across our client's Support at Home Program, leading a team of Care Partner Registered Nurses, Enrolled Nurses, Clinical Care Partners and Care Partners. You'll be the clinical anchor for complex cases, ensuring every client receives safe, coordinated, goal-focused care that helps them maintain or regain independence.
While the role isn't designed to carry a full caseload, you'll stay close to practice — stepping in to cover leave and supporting your team through complex clinical decisions.
What you'll be doing
- Providing clinical leadership across the Support at Home Program, guiding complex cases and embedding reflective practice
- Overseeing care planning, referrals and multidisciplinary coordination across nursing,
allied health and assistive technology
- Leading, mentoring and developing a multidisciplinary clinical team
- Driving clinical governance, quality and compliance in line with the Strengthened Aged Care Quality Standards
- Overseeing client budgets and funding within delegated authority
- Building strong relationships with clients, families and external stakeholders, including GPs, hospitals and Services Australia
What you'll bring
- Registered Nurse qualification with current AHPRA registration
- Strong knowledge of the Support at Home Program and Strengthened Aged Care Quality Standards
- Proven experience in clinical assessment, case management and goal-based care planning
- Demonstrated leadership experience, with the ability to build a positive, high-performing team culture
- Excellent communication skills and confidence working with clients, families and external stakeholders
- Current driver's licence, reliable vehicle and comprehensive insurance
