Western Health is seeking a Grade 2 Clinical Pharmacist – Transitions of Care to join our Pharmacy team. The role focuses on improving medication safety as patients move between hospital, home and other settings, working with patients, carers and multidisciplinary teams to ensure accurate and transparent medication information.

You will perform medication reconciliation, resolve discrepancies, provide evidence-based recommendations and educate patients, with communication to GPs and community
