We are seeking a dedicated and experienced HACC PYP Coordinator to join our team at Community Accessibility in Wangaratta. As the HACC PYP Coordinator, you will play a pivotal role in our Aged Care division, overseeing and coordinating key operations to ensure the delivery of high-quality services to our clients. Your responsibilities will include:
- Managing and coordinating the Home and Community Care (HACC) Program for Younger People (PYP) in accordance with legislative requirements and organisational policies
- Assessing the needs of clients and developing individualised care plans to support their independence and wellbeing
- Overseeing a team of support staff, providing guidance, training, and supervision to ensure exceptional service delivery




- Liaising with external service providers, stakeholders, and community organisations to facilitate access to additional support services for clients
- Collaborating with internal departments to streamline processes and improve overall operational efficiency
- Maintaining accurate records, reports, and documentation in compliance with regulatory standards
- Participating in continuous quality improvement initiatives to enhance the effectiveness of the HACC PYP program
- Contributing to the development of policies, procedures, and strategies to meet the evolving needs of our client base

- Proven experience in coordinating Home and Community Care (HACC) Program for Younger People (PYP)
- Demonstrated understanding of the aged care sector, including relevant legislation and funding arrangements
- Strong organisational and project management skills, with the ability to prioritise tasks effectively
- Excellent communication and interpersonal skills, with the ability to engage with diverse stakeholders
- Ability to develop and implement service delivery plans that meet the needs of younger people requiring support
- Experience in leading and supervising a team, fostering a collaborative and supportive work setting
- Proficiency in using relevant software applications for data management and reporting
- A commitment to promoting accessibility, inclusion, and independence for younger people with disabilities or complex needs
- Relevant qualifications in community services, health, social work, or a related field
- Current knowledge of best practices and emerging trends in HACC PYP service delivery
