Where you'll be working
The Mount Druitt Aboriginal Health Hub, located at Mount Druitt Hospital, is dedicated to improving the health and wellbeing of Aboriginal and Torres Strait Islander peoples through culturally safe, community-focused care. Working within Western Sydney Local Health District, you'll be part of a passionate team committed to strengthening connections between Aboriginal communities and healthcare services.

What you'll be doing
- Support Aboriginal people and their families to access community, primary, specialist and hospital-based chronic care services
- Build trusted relationships and develop referral pathways that improve access to coordinated care and support
- Advocate for culturally safe healthcare practices and enhance service capability to meet the needs of Aboriginal communities
- Deliver education, health promotion, community engagement and practical support for people living with chronic and complex conditions
- Foster collaboration between WSLHD services, Aboriginal Community Controlled Organisations, government agencies and community partners
Essential Criteria
- License: Driver’s licence valid in NSW
- Qualification: Holds or is willing to undertake a minimum Certificate III qualification in Aboriginal Health Care or other relevant qualification s (including undergoing recognition of prior learning processes against current qualifications)
- Evidence of Diversity claim if applicable (state which category): Aboriginal and / or Torres Strait Islander
Selection Criteria
1. This is an identified Aboriginal Position. Applicants must be of Aboriginal and / or Torres Strait Islander descent, have demonstrated knowledge and understanding of Aboriginal and / or Torres Strait Islander cultures and have established links with their local Aboriginal community. An applicant’s race is a genuine occupational qualification and exemption is claimed under Section 14d of the Anti-Discrimination Act 1977, NSW.
2. Holds or is willing to undertake a minimum Certificate III qualification in Aboriginal Health Care or other relevant qualifications (including undergoing recognition of prior learning processes against current qualifications)
3. Experience in NSW Aboriginal Health Programs and / or experience in providing social support, linkage to services, group work, community education, and health promotion with the Aboriginal community

Demonstrated understanding of issues impacting on Aboriginal and Torres Strait Islander people and their carers / families and the impact that chronic and complex conditions may have
5. Demonstrated capacity to lead the development of culturally safe and respectful environments for Aboriginal clients / patients and their families
6. Demonstrated ability to provide cultural brokerage with Aboriginal people and communities; cultural consultancy for other members of the team; and to facilitate respectful and culturally safe access to services.
7. Demonstrated ability to work autonomously and as a member of a multidisciplinary team with effective interpersonal, problem solving and computer skills
8. Current drivers licence valid in NSW
