Position Overview:

We are seeking a motivated and proactive Community Worker to join our team. This role focuses on community engagement, coordination and empowerment rather than personal care. You will play a key part in helping participants navigate services, strengthen their independence and stay socially connected.

The ability to work independently with professionalism and empathy

A flexible, proactive, and adaptable approach to a dynamic work environment

Experience in health, welfare, or community services (highly regarded)

Qualifications in Community Services, Social Work, Welfare, or related fields (highly regarded)

You will be required to:

Lead initiatives to obtain grants and funding resources for Heartcare





Evaluate local needs and available resources across health, welfare, housing, employment, education, and related services

Coordinate with community organisations, welfare providers, government agencies, and private sector partners to address community concerns and increase awareness of available support and initiatives

Identify client requirements and design, deliver, and manage targeted education, training, and support programs

Provide practical assistance, emotional support, and advocacy to individuals and families

Assist clients with access to community resources and social participation opportunities

Work respectfully and professionally under general guidance while managing your own time effectively

Maintain accurate records, exercise sound judgment, and uphold confidentiality and sensitivity in all interactions

Facilitate client engagement in community activities and programs, supporting participation in schemes like the National Disability Insurance Scheme (NDIS) to foster social inclusion and personal growth

Reflect HeartCare’s values of compassion, integrity, and empowerment in all aspects of service delivery

What we need from you:

An associate degree, Advanced Diploma, Diploma, or equivalent qualification in Community Services, Social Work, or a related field

Relevant experience of at least three (3) years work experience may substitute for qualifications above





A current Working with Children Check and National Police Check

A current First Aid Certificate

A valid Driver’s License and reliable vehicle

Two professional referees
