Sale and 62 kilometers south east of Traralgon.

What’s on offer The Leisure and Lifestyle Coordinator – Planned Activity Group (PAG) is responsible for the coordination, planning, delivery and review of the Planned Activity Group and related leisure and lifestyle programs for aged, disabled and/or socially isolated members of the community. The role promotes physical, intellectual, emotional, cultural and social wellbeing, independence, choice,

community participation and quality of life through personalised individual and group activities,

programs and outings.

The Coordinator works within a wellness and reablement, person-centred approach and ensures that the PAG operates in accordance with relevant legislation, aged care and community care requirements,

YDHS policies and procedures, quality standards, privacy and confidentiality requirements,



and work health and safety obligations.

About you To be successful in the role, you will need:

- Certificate III or IV in Leisure and Lifestyle, Aged Care, Diversional Therapy or a similar

relevant qualification, or willingness to obtain appropriate training.
- Demonstrated experience in developing, coordinating and delivering individual and group

social, recreational, leisure, lifestyle, diversional and/or intellectual activities for older people and/or people with disability.
- Experience working with clients who may be frail, aged, disabled or socially isolated, with an

understanding of person-centered and wellness approaches.
- Demonstrated ability to coordinate programs, organise activities and outings, prioritise

competing requirements and work effectively with limited supervision.
- Demonstrated ability to work collaboratively as part of a multidisciplinary/community care

team, including taking direction and using initiative.
- Robust verbal and written communication and interpersonal skills.
- Competence with Microsoft Office, including Outlook, Word and Excel, and ability to complete

required documentation and data entry.
- Current Victorian Driver’s License.
- Current First Aid Certificate.

- An original National Police Record Check completed within the last twelve (12) months prior

to commencement.
- An employee Working with Children Check.
- Relevant professional registrations.
- Complete a Pre-existing injury declaration form.
- Undergo any other relevant checks, education or licencing as directed at own cost.
