About the role

Depending on your experience, you will help participants understand and implement their NDIS plans, build participant capacity, choice and control, and coordinate services across disability, health, housing, justice and community systems. You will identify and respond to risks, service gaps and safeguarding concerns, support participants through reviews, changes in circumstances and periods of crisis or transition, build effective relationships with participants, families, nominees, guardians, providers and government agencies, maintain explicit, timely and professional case notes and documentation, and work collaboratively while remaining firmly focused on the participant's goals, rights, will and preferences. Our work includes both Level 2 Support Coordination and Level 3 Specialist Support Coordination.

Key responsibilities

- Help participants understand and implement their NDIS plans
- Build participant capacity, choice and control
- Coordinate services across disability, health, housing, justice and community systems
- Identify and respond to risks, service gaps and safeguarding concerns
- Support participants through reviews, changes in circumstances and periods of crisis or transition
- Build effective relationships with participants, families, nominees, guardians, providers and government agencies
- Maintain clear, timely and professional case notes and documentation
- Work collaboratively while remaining firmly focused on the participant's goals, rights, will and preferences

About you

- Have experience in NDIS Support Coordination,



Specialist Support Coordination or a related complex-practice role
- Understand the NDIS and can confidently navigate its processes and systems
- Can manage competing priorities without losing sight of the person at the centre
- Communicate clearly and build trust with people from a wide range of backgrounds
- Are comfortable advocating when systems are not working as they should
- Use sound professional judgement and know when to seek guidance
- Value ethical, independent practice and understand why conflicts of interest matter
- Enjoy being part of a team that shares knowledge, challenges ideas and supports one another
- Meet relevant NDIS worker-screening and other statutory requirements

Coordination is all we do, we do not deliver SIL, personal care, therapy, accommodation or other direct supports. That independence matters. It allows our team to support people to navigate complex systems genuinely without competing priorities, focus entirely on the participant and avoid the conflicts that have become far too common across the sector. We support approximately 1,600 NDIS participants across NSW, with a high concentration of people navigating complex health, mental health, housing, justice, guardianship and safeguarding systems. We are clinically led, directly employ our team and are often asked to become involved when situations are already complex, vulnerable or at risk of breaking down.
