About SAN Support
SAN Support is a growing NDIS provider committed to helping people with disability achieve greater independence, choice and control. We provide participant-focused services and work closely with families, providers and community organisations to achieve meaningful outcomes. We are seeking an experienced and compassionate NDIS Support Coordinator to join our team.
About the Role
As a Support Coordinator, you will support NDIS participants to understand and implement their plans, connect with suitable providers and build their confidence to manage their supports. You will maintain positive relationships with participants, families, support providers, allied health professionals and other stakeholders.
Key Responsibilities
- Help participants understand and implement their NDIS plans.
- Identify suitable services and connect participants with quality providers.
- Build participants’ capacity to manage their supports independently.
- Coordinate services across disability, health, housing and community sectors.
- Monitor participant budgets and ensure effective use of NDIS funding.
- Conduct regular participant reviews and respond to changing support needs.
- Prepare progress reports, review reports and supporting documentation.
- Assist participants with NDIS plan reassessments and change-of-circumstances requests.
- Manage risks, service gaps, incidents and complex situations appropriately.
- Maintain accurate case notes and records in accordance with NDIS requirements.
- Develop strong relationships with participants, families, providers and community organisations.
- Build referral networks and contribute to the continued growth of SAN Support.
- Maintain appropriate professional boundaries and participant confidentiality.
About You
The successful candidate will ideally have:
- Previous experience in NDIS Support Coordination or a similar role.
- A good understanding of NDIS plans, budgets and funding categories.
- Knowledge of the NDIS Practice Standards and participant safeguarding requirements.
- Strong communication, relationship-building and problem-solving skills.
- The ability to manage a participant caseload and meet reporting deadlines.
- Strong organisational and time-management skills.
- Confidence working independently and as part of a team.
- Experience supporting participants with complex needs will be highly regarded.
- Relevant qualifications in disability, community services, social work, psychology, allied health or a related area will be advantageous.
Essential Requirements
- Current NDIS Worker Screening Check.
- Working with Children Check, where required.
- Current Australian driver’s licence.
- Access to a reliable and insured vehicle.
- Working rights in Australia.
- Ability to complete the NDIS Worker Orientation Module.
- Satisfactory reference and employment checks.
