About the role

As a Senior Disability Support Mentor, you will provide direct 2:1 support while also helping maintain the quality, consistency and stability of the support team as needed. This is an opportunity to become a key person in the life of one individual with complex support needs, providing highly individualised support to one person living with autism, epilepsy and intellectual disability. Your focus is one person — understanding them, knowing what matters to them, building trust and rapport, and helping them live the best life possible.

Key responsibilities

- Provide hands on high-quality 2:1 individualised support
- Build and maintain trust, rapport and a genuine professional relationship with the participant
- Take the time to understand the participant's communication, preferences, interests, routines and individual needs
- Help coordinate the individualised day program
- Support community participation and meaningful activities
- Support the participant in his microbusiness activities
- Encourage independence, confidence and skill development and capacity building
- Work alongside the Team Leader and Operations Supervisor
- Work collaboratively with allied health professionals
- Provide delegated leadership when the Team Leader is unavailable
- Help ensure routines, support strategies and goals are consistently implemented

About you

- Demonstrated experience in disability support
- Strong experience with complex support needs
- Experience working in a 2:1 support environment
- Prior experience mentoring and training others
- Current NDIS Worker Screening Check
- NDIS Worker Orientation Modules completed




- Current First Aid and CPR
- Medication competency
- Midazolam/epilepsy competency
- Food Handling Certificate
- Current driver's licence
- Understands positive behaviour support, can understand and implement Behaviour support plans and person-centred practices
- Can remain calm and capable when situations become challenging
- Has strong communication and leadership skills
- Is organised, reliable and accountable
- Trustworthy, has empathy and is kind and sincere in all interactions with participants
- Works with maturity and sound judgement
- Wants to build a long-term relationship with the person they support

About us Team Jake (Vic) Ltd provides highly individualised support to one person living with autism, epilepsy and intellectual disability. Our service operates on a 2:1 support model, providing two support workers to one participant at all times. We are looking for exceptional people who want to provide exceptional support and become part of a very important team delivering services within a Service For One unique model of support.
