The Royal Melbourne Hospital is seeking an experienced Clinical Aged Care Assessor to join the Aged Care Assessment team at the Royal Park campus. You will conduct strengths-based assessments across hospital, community, home and telehealth settings to tailor care plans for older Australians.
Collaborate with families, care providers and health specialists to coordinate services, review plans and advocate for patient rights. A current driver’s license and ability to travel are required.
