Are you a compassionate and proactive nurse looking for a flexible role within a supportive and collaborative team? Synergy Medical Practice is an independently owned and operated general practice located in the heart of St Leonards on Sydney's Lower North Shore. We are committed to delivering high-quality, patient-centred care and are seeking an experienced Practice Nurse to join our friendly team.

About the Role We are offering a part-time position of approximately 10 hours per week , with flexibility to split the hours across two days . Some duties may also be performed remotely, providing the opportunity to work from home for part of your contracted hours.

Key Responsibilities
Chronic disease management and care plan coordination
Health assessments and recalls
Immunisations and vaccine management
Patient education and health promotion
Assisting with quality improvement activities
Clinical triage and patient follow-up
Management of recalls and reminders
Support with accreditation and compliance activities




General nursing support to our GPs and clinical team

Requirements / Qualifications
Current AHPRA registration as a Registered Nurse
Experience in general practice nursing (preferred)
Strong organisational and communication skills
A patient-focused approach and commitment to high-quality care
Experience with chronic disease management and preventative health programs
Ability to work both independently and collaboratively within a small team
Proficiency with clinical software systems and Microsoft Office applications
