Job Description

Nepean Hospital, part of the Nepean Blue Mountains Local Health District, is seeking a qualified Social Worker to join Renal Services. The role involves psychosocial assessment, intervention and care coordination for patients and families, within a multidisciplinary team, with a high degree of autonomy.

You will apply clinical reasoning, crisis intervention and case management skills to deliver person-centred care and support continuity of care across the patient journey.
