About the Role

as a Social Groups Support Worker, assisting in the delivery of high-quality support and engaging activities for older adults attending 3Bridges social groups, outings, and shopping trips.
In this role, you will support clients' independence, wellbeing, and social engagement through practical assistance, personal care, and active participation in group activities.
You'll work collaboratively with the Team Leader and wider 3Bridges team to ensure each client enjoys a protected, inclusive, and stimulating environment.
This position is ideal for someone compassionate, energetic, and passionate about supporting older adults or vulnerable community members to live fulfilling, connected lives.
Flexibility to work across all sites is required.
Key Responsibilities
Assist in the planning and delivery of group programs, recreational activities, and outings.
Support clients with personal care, hygiene, and functional needs.
Encourage participation and engagement in group discussions and activities.
Monitor and report changes in clients' functional abilities or wellbeing.
Provide feedback to inform program development and individual activity plans.
Maintain accurate and timely client records and documentation.
Support home care services when groups are not running, including domestic assistance and social support.
Promote the service to the community and support continuous improvement initiatives.
Drive a small 3Bridges van as required (full licence sufficient; LR licence highly desirable).
Skills & Qualifications
Minimum 1 year experience working with frail, aged, or vulnerable people, or those with disability.
Certificate III in Individualised Support / Aged Care, or equivalent.
Manual handling experience.




Strong understanding of the Aged Care Quality Standards.
Excellent communication and interpersonal skills.
Ability to work sensitively and respectfully with vulnerable clients.
Experience collaborating within a multidisciplinary team.
Experience supporting individuals with dementia will be highly regarded
