About the Role
We're looking for an experienced and compassionate Grade 2 Social Worker to join our Community Rehabilitation team in a part time, ongoing position, predominantly based at Kingston Centre in Heatherton.
Community rehabilitation is about more than supporting someone through an episode of care.
It's about understanding the person behind the diagnosis - their home, family, relationships, responsibilities and the things that matter most to them - and helping them move forward with confidence and the right supports around them.
Working within an experienced interdisciplinary team, you'll provide short-term, goal-focused Social Work intervention to adults across a diverse range of clinical streams, including neurological, musculoskeletal, general, cardiac and pulmonary rehabilitation.
Your work will include:
Providing psychosocial assessment, intervention, case management and discharge planning
Supporting clients and their families or carers to navigate change, adjustment and complex circumstances
Working with clients where family violence may be suspected or disclosed
Connecting people with community services, supports and funding pathways
Collaborating closely with the broader interdisciplinary team to achieve meaningful client goals
Liaising with hospital and community-based services to support continuity of care




Providing clinical supervision to Grade 1 Social Workers and Allied Health Assistants, as allocated
Supporting the learning and development of Social Work students
Contributing your ideas and experience to service development, evidence-based practice and quality improvement
About Community Rehabilitation
Monash Health Community Rehabilitation helps people improve and maintain their health, independence and wellbeing following illness, injury or a change in their health.
Our care is delivered where it works best for the client - at home, at one of our Community Rehabilitation centres or through tele-rehabilitation - and may include individual or group-based intervention.
Our approach is time-limited, goal-focused and tailored to the individual.
We look beyond a person's diagnosis to understand what they want to get back to doing and what might be standing in their way.As a Social Worker, you'll bring an important perspective to that journey, helping clients and families navigate the social, emotional and practical impacts of changes to their health while connecting them with the supports they need to continue living well in their community.
About You
You're a Social Worker who enjoys variety, collaboration and seeing the real-world impact of your work.You'll bring:
Experience providing high-quality, person-centred Social Work assessment and intervention
Strong clinical reasoning and the ability to confidently manage a varied caseload
An understanding of trauma-informed practice and working with vulnerable populations
An awareness of the social, cultural, legal and systemic factors that can influence a person's health and wellbeing
Strong organisation, prioritisation and time-management skills
A collaborative approach and willingness to contribute as part of an interdisciplinary team
The ability to reflect on your practice and continue developing your clinical skills
A genuine commitment to helping people maintain independence and participate meaningfully in their lives and communities
