CareLinc is seeking an empathetic NDIS & Aged Care Case Manager to work in office in South-West Sydney (Liverpool / Fairfield West). You will coordinate care plans, oversee service delivery and ensure compliance with NDIS and Aged Care standards.
You will engage with families and health professionals, maintain documentation, and support participants and home care clients through coordinated services. A bilingual advantage in Arabic/Vietnamese/Spanish is highly regarded.
