Aboriginal Health Worker - La Perouse Aboriginal Outreach Clinic

The Role

The Aboriginal Health Worker:

- Provides support and care coordination to all Aboriginal and Torres Strait Islander people referred to La Perouse Aboriginal Outreach Clinic, working in partnership with the clinical care providers in Community Health;
- Implements a range of health promotion activities;
- Referral and scheduling of clients into relevant clinics, supporting clients with coordination of referrals, tests and transports to clinics;
- Day to day support of clinics in collaboration with SESLHD clinicians;
- Participates in the review and development of existing and new programs;




- Provides support to Aboriginal and Torres Strait Island clients referred to Community Health
- Improving culturally appropriate care in Prince of Wales for Aboriginal Adults/families by providing support and advocacy, supporting client's health care journey and identifying any barriers and enhancing health outcomes

Where you'll be working

Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including cutting-edge virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person-centred care.

Selection Criteria

- Being of Aboriginal &/or Torres Strait Islander descent is a genuine occupational qualification for this position under Section 14 of the Anti-Discrimination Act 1977 (NSW).
- Holds or aspires to hold a minimum Certificate III in Aboriginal Primary Health Care




- Knowledge and understanding of significant health issues that impact on the health and wellbeing of Aboriginal people and their families/communities
- Demonstrated experience in health promotion/education, and coordination of program delivery as an Aboriginal health worker
- Experience and knowledge of Microsoft Office Applications
- Demonstrated oral and written communication skills and ability to work as part of a team
- Current NSW Drivers Licence Class C
