About the role
As a Diabetes Nurse Educator (DNE), you will provide specialist clinical care and support to people with acute and chronic diabetes within a community setting, working collaboratively with clients, families, GPs and multidisciplinary teams.

Your duties include:
- Deliver high quality care Diabetes Nursing service and conduct assessments with a particular focus on the adult /aged population
- Conduct assessments and deliver interventions guided by best-practice principles
- Provide secondary consultation where appropriate, contributing to multidisciplinary case discussions
- Promote access to Diabetes Education by providing outreach or off-site services as required

Our ideal candidate:
- Registered Nurse, Division 1 with relevant AHRPA Registration and ADEA Diabetes Educator credentials
- Experience working in a multi-disciplinary team
- Experience developing and maintaining strong networks, including external partners
- Victorian Driver’s License
