About the role

Playing a vital role in the transition from hospital to community care, this position supports consumers with complex mental health and alcohol and other drug treatment needs as they move from acute inpatient services back into the community. Working within the Redcliffe Caboolture Transitional Care Team, you will help ensure consumers receive timely follow-up care, are connected to appropriate ongoing supports and achieve the best possible psychosocial outcomes.

This role combines clinical social work practice, advocacy and multidisciplinary collaboration to support recovery-focused care. Through comprehensive assessment, evidence-based interventions and strong partnerships with consumers, families, healthcare providers and community agencies, you will contribute to safe discharge planning, reduced hospital length of stay and improved continuity of care.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

- Deliver evidence-based social work services to consumers with complex mental health and alcohol and other drug treatment needs, using recovery-oriented and trauma-informed approaches.
- Conduct comprehensive biopsychosocial assessments, risk assessments and targeted interventions that support recovery, wellbeing and community reintegration.
- Collaborate with consumers, carers, multidisciplinary teams,



government services and community organisations to facilitate coordinated care and effective discharge planning.
- Manage a complex caseload while exercising sound clinical judgement, problem-solving skills and appropriate risk management strategies.
- Contribute to quality improvement, service development, student education and professional supervision activities that enhance social work practice and consumer outcomes.
