Life Without Barriers is seeking a Care Partner to coordinate high-quality aged care services across Ballina, Lismore and surrounding areas. You will manage referrals, complete assessments, develop and review care plans, and conduct home visits to ensure personalised support.

The role is permanent full time, based in Ballina with regular travel throughout the region. You can work some days from home and at our Ballina office, offering a meaningful path in aged care with real community impact.
