Job Description
UnitingCare Queensland is seeking a Supported Living Facilitator (Disability Services, NDIS) in Worongary and Parkwood on a casual basis. You'll work with the person supported, families and networks to help achieve life goals.

Responsibilities include assisting with daily living, personal care, developing and reviewing individual plans, promoting autonomy and rights, privacy and dignity, with versatile schedule and English communication.
