The Institute for Urban Indigenous Health (IUIH) is seeking a Social Health Care Coordinator to provide coordinated supports for Aboriginal and Torres Strait Islander clients with complex social and mental health needs.
You will work with a multidisciplinary team at the Goodna clinic to access services and supports, ensuring optimal client outcomes.
Day-to-day duties include triage, case management via MMex, and contributing to care reviews with counsellors, psychologists and other team members.
