Encircle Community Services is a leading community-based organisation supporting children, young people and families across the Moreton Bay region. For almost 40 years, we have provided quality support services that promote wellbeing, resilience and connection within our community.
We are currently seeking a passionate and motivated Family Support Practitioner to join our Families Team on a full-time, 3-month contract. Based from our Lawnton site, this role involves both office-based and outreach support to families across the Moreton Bay region.
This is an excellent opportunity for someone who is committed to working alongside families to build capacity, strengthen relationships and achieve positive outcomes for children and young people.
About the Role
As a Family Support Practitioner, you will provide strengths-based, trauma-informed support to families experiencing a range of challenges, including parenting concerns, family conflict, domestic and family violence, mental health impacts, financial hardship and social isolation.
Working collaboratively with families and community partners, you will assist clients to identify goals, access supports, strengthen resilience and improve family wellbeing.
Key Responsibilities
- Provide family support and case coordination services using strengths-based and child-centred approaches.
- Work collaboratively with families to develop and review support plans and goals.
- Undertake assessments and identify appropriate supports and referral pathways.
- Maintain accurate case notes, records and reporting requirements.
- Advocate for families and assist them to navigate community services.
- Facilitate and support group programs and community activities.
- Build effective relationships with local stakeholders and partner organisations
- Contribute positively to a collaborative and supportive team environment. About You
You are an empathetic practitioner who is passionate about helping families thrive and understands the importance of early intervention and family-centred support.
To be successful, you will have:
- A qualification in Social Work, Human Services, Psychology, Social Science or a related discipline.
- Experience working with children, young people and families in a community services setting.
- Knowledge of strengths-based, trauma-informed and family-focused practice.
- Strong communication, relationship-building and organisational skills.
- The ability to work independently while contributing to a multidisciplinary team.
- Experience engaging with diverse communities, including First Nations peoples and culturally and linguistically diverse communities.
Essential Requirements
- Current Queensland Driver Licence.
- Current Blue Card.
- National Police Check.
- Current First Aid and CPR certification or ability to obtain.
