Lifestyle Solutions is seeking a Youth & Family Case Manager to support children and young people in Out-of-Home Care.
The role combines case management, therapeutic planning, and frontline coaching with trauma-informed, culturally protected practices.
You will split your time between guiding frontline staff (18 hours per week) and completing case work duties (20 hours per week), including planning, documentation, and stakeholder engagement in Tamworth, NSW.
