POSITION OVERVIEW
The Community Aged Care Nurse delivers nursing care directly in clients' homes and community settings, supporting older clients to remain safe, well, and independent. This is initially offered as a contract position, with the opportunity to transition to full-time employment in the future depending on caseload growth and operational needs. The role involves building strong relationships with clients and families, coordinating with other allied health professionals, and adapting care delivery to each client's home workplace.
KEY RESPONSIBILITIES
Conduct nursing assessments and health monitoring in clients' homes
Develop and review individualised care plans in consultation with clients, families, and the broader care team
Administer medications and clinical treatments as required
Provide wound care, continence care, and other clinical nursing tasks as needed
Identify and elevate changes in client health status promptly
Maintain accurate, timely clinical documentation for each visit
Support clients and families to understand and manage ongoing health needs
Plan and manage a caseload of home visits across a designated area
QUALIFICATIONS & REGISTRATION
Bachelor of Nursing (or equivalent)
Current registration with AHPRA (Nursing and Midwifery Board of Australia)
Working with Children Check
NDIS Worker Screening Check
Current Police Check
SKILLS & EXPERIENCE
Experience in community, home-visiting, or aged care nursing
Confidence working independently and managing a home-visit schedule
Robust interpersonal skills for building rapport with older clients and families
Good time-management skills to coordinate visits across a geographic area
Current Australian driver's licence and access to a reliable vehicle
