Synergy Community Services Pty Ltd is seeking compassionate, reliable and motivated Disability Support Workers to join our growing team. You will provide person-centred support to NDIS participants in their homes and the community, assisting with daily living, personal care, community participation and independence.
Key responsibilities
Personal care, showering, dressing and grooming
Assistance with daily living and household activities
Meal preparation and domestic assistance
Community access, appointments and social activities
Mobility and transfer support
Supporting participant independence and choice
Maintaining accurate shift/progress notes
Reporting incidents, hazards and changes in participant needs
Following NDIS requirements, participant support plans and organisational policies
About you
Current Australian Driver's Licence – Mandatory
Reliable, registered and comprehensively insured vehicle – Mandatory
NDIS Worker Screening Clearance
First Aid & CPR certificates
Australian working rights
Ability to complete required background and reference checks
Valuable communication and documentation skills
Commitment to participant rights, dignity, privacy and choice
Ability to work independently and as part of a team
Certificate III/IV in Individual Support, Disability, Community Services or related field (desirable)
