About the Role
As a Community Home Care Support Worker, you will provide personalised support to clients living in their own homes, helping them maintain their independence and quality of life.
Your Responsibilities
Assist with personal care, including showering, dressing, grooming, and mobility support
Support daily living activities such as meal preparation, medication reminders, shopping, and light household tasks
Provide companionship and social support
Assist clients to attend appointments and participate in community activities
Promote independence, choice, and dignity in all aspects of care
Maintain accurate documentation and progress notes




Build positive relationships with clients, families, and healthcare professionals
About You
You are caring, dependable, and passionate about supporting others.
You'll bring:
Certificate III or IV in Individual Support, Aged Care, Home and Community Care, Disability Support, or a related qualification (or currently studying)
Previous experience in community care, home care, aged care, disability support, or a similar role
Excellent communication and interpersonal skills
A compassionate and client-focused approach
Strong time management and organisational skills
Ability to work independently and as part of a team
Essential Requirements
Current National Police Check
Current First Aid and CPR Certificate
Current Working with Children Check (or willingness to obtain)
Valid Australian Driver's Licence
Reliable and comprehensively insured vehicle
NDIS Worker Screening Check (desirable)
