Job Description
BlueCare, part of UnitingCare Queensland, is seeking a Care Partner to deliver person-centred community care in Logan and surrounding areas.
You will work with clients, coordinate services, and support multidisciplinary teams to implement care plans across the client's aged care funding.
The role is field-based, requiring driving, robust communication, and the ability to work autonomously.
Qualifications include a Certificate IV in Health or Community Service, a valid Australian driver's
