Job Description
Wesley Mission Queensland Limited is seeking a Family & Community Support Practitioner for Hummingbird House.
In this part-time role (30 hours/week) you will provide psychosocial and emotional support to children and families facing reliable illness, end-of-life care and bereavement, within a multidisciplinary team and family-centred care framework.
You will conduct assessments, develop tailored plans, deliver grief and bereavement support and collaborate with healthcare and community partners to
