Job Description
Interchange Australia is seeking a Clinical Aged Care Needs Assessor to perform face-to-face assessments in clients' homes, determining eligibility for government-subsidised aged care services and supporting decision-making for care options.
You will conduct assessments, maintain knowledge of My Aged Care and related processes, and educate clients on eligibility and referrals, while coordinating with community organisations to secure necessary home supports.
