TeamHEALTH in Darwin is seeking a Housing Coach to support tenants exiting homelessness, helping them stabilise, gain independence, and secure long-term housing across social, affordable, or private rentals.
You will work with people with complex needs in a versatile, person-centred approach.
You will manage a caseload, develop individual action plans, and connect clients to appropriate supports.
The role requires rotating rosters including evenings and weekends and collaboration with tenants,
