Job Description
Circle of Hope is seeking an Aged Care Intake Specialist to support clients in Sydney, NSW, with in-home aged care services.
You will assess needs, plan services, and coordinate supports with families and healthcare providers to ensure person-centered care.
In this role you will document assessments, manage schedules, and liaise with care partners to deliver timely, compliant home care support across the region.
You will work closely with supervisors to ensure quality and regulatory compliance.
