Embrace Community Care is seeking a motivated, caring and reliable person to join our team as a Team Leader in the Picton NSW.

This is an excellent prospect for someone who is passionate about supporting people with disability and is looking to develop their leadership and coordination skills within a supportive community services environment.

About the Role

As a Team Leader, you will help coordinate the day-to-day operations of our services and support our Support Workers to deliver high-quality, person-centred support.

Your responsibilities will include:
Supporting and guiding Support Workers
Assisting with day-to-day service coordination
Communicating effectively with participants, families and staff
Helping ensure supports are delivered according to participant needs and preferences
Supporting staff with day-to-day questions and concerns
Maintaining accurate records, documentation and shift information
Promoting a positive, respectful and inclusive environment
Following Embrace Community Care policies, procedures and NDIS requirements
Identifying and escalating concerns, incidents and issues to management when required




Supporting effective communication between staff, participants and management
Helping maintain a secure and supportive environment for participants and staff

About You

Organised and able to manage multiple tasks
Comfortable working with people with disability and their support teams
Willing to learn and develop leadership skills
Passionate about supporting people with disability
Respectful of participant choice, dignity, rights and independence
Able to work independently and take initiative when required
Professional and committed to providing high-quality support
