Vista Life Innovations is seeking a Case Coordinator (Case Manager) to provide direct support and case coordination for Engage members in Madison, CT. The role maintains an assigned caseload and serves as the primary contact for members, families, guardians, support teams, and community partners.
Approximately 30 hours per week are dedicated to direct service, with remaining hours for administrative duties, supervision, documentation, meetings, and family communication.
